Patient Engagement (Mental Health Services)
We are a rapidly growing mental health service provider dedicated to improving the quality of life for seniors and vulnerable adults. We are seeking a highly motivated and empathetic Outreach & Client Engagement Specialist to serve as the vital link between our services and the clients within our partner Adult Day Care Centers.
The ideal candidate is an enthusiastic "people person" with a genuine passion for connecting with others, educating them about mental wellness, and facilitating their access to necessary care.
Key Responsibilities
1. Client Education & Outreach
Regular Site Visits: Consistently visit assigned Adult Day Care Centers to establish a regular presence and build trust with center staff and clients.
Informational Sessions: Conduct engaging and accessible group presentations, workshops, and informal one-on-one discussions to educate clients and their families about the benefits and specifics of our mental health services.
Service Promotion: Clearly articulate the value proposition of our services, addressing common concerns and overcoming barriers to participation.
2. Enrollment & Client Intake
Sign-Up Facilitation: Guide interested clients through the initial enrollment and sign-up process for our services, ensuring all necessary documentation is completed accurately and confidentially.
Needs Identification: Conduct preliminary, respectful conversations with potential clients to understand their needs and determine if our services are an appropriate fit.
Coordination: Act as the primary liaison, coordinating with our internal clinical team and the Adult Day Care Center staff to ensure a smooth transition for new clients starting services.
3. Relationship Management
Trust Building: Develop and maintain strong, positive working relationships with the administrators, social workers, and staff at the Adult Day Care Centers.
Feedback Loop: Collect and relay client and staff feedback to our management team to help continuously improve service delivery and outreach strategy.
4. Reporting & Administration
Tracking: Maintain accurate, detailed records of all outreach activities, including centers visited, presentations given, and client sign-ups achieved.
Goal Achievement: Work toward weekly and monthly client engagement and enrollment targets.
Qualifications & Requirements
Experience: Proven experience (2+ years preferred) in community outreach, sales, marketing, client relations, or a similar role, ideally within healthcare, social services, or senior care.
Education: A high school diploma or equivalent is required; an Associate's or Bachelor's degree in a related field (e.g., Social Work, Psychology, Communications) is a plus.
Travel: Must have reliable transportation and the ability to travel regularly between assigned Adult Day Care Centers.
