Mindify Wellness and Care is seeking a compassionate, outgoing, and bilingual Patient Care Coordinator to help connect older adults with behavioral health services that can improve their quality of life.
This is a relationship-focused role for someone who enjoys working directly with people, educating communities, and making a meaningful impact. You'll spend your time visiting adult day care centers, building trust with participants and families, providing information about available mental health services, and helping individuals navigate the care they need.

Compensation & Benefits
Hourly Rate: $23–$25 per hour

What You'll Do
- Visit assigned adult day care centers throughout Northern and Central New Jersey.
- Educate participants, families, and staff about Mindify's psychiatric, therapy, and behavioral health services.
- Build strong, trusting relationships with adult day care participants and caregivers.
- Help identify individuals who may benefit from behavioral health support and facilitate referrals.
- Assist patients in understanding available treatment options and accessing services.
- Support patient engagement and continuity of care.
- Serve as a liaison between patients, families, providers, and adult day care staff.
- Provide Spanish-English interpretation and translation when needed.
- Collaborate with center administrators, activity coordinators, and care teams.
- Maintain accurate documentation of outreach activities, referrals, and patient interactions.
- Uphold HIPAA standards and maintain patient confidentiality.
- Perform additional patient advocacy and support activities as needed.
What We're Looking For
Required Qualifications
- Fluent in both English and Spanish.
- Excellent communication and interpersonal skills.
- Comfortable speaking one-on-one and presenting to small or large groups.
- Strong relationship-building and advocacy skills.
- Valid driver's license and reliable transportation.
- Willingness to travel throughout Northern and Central New Jersey.
- Organized, dependable, and self-motivated.
- Proficiency with Microsoft Office and electronic documentation systems.

Experience in any of the following areas is helpful but not required:
- Patient services
- Healthcare
- Behavioral health
- Social services

- Patient advocacy

Schedule & Work Environment
This is a part-time position with most hours occurring during standard weekday daytime hours.
- Typical schedule: Monday through Friday
- Generally between 8:30 AM and 2:30 PM
- Up to approximately 30 hours per week
- Daily schedules may vary based on community needs, patient engagement activities, and travel requirements
This role is primarily community-based and involves regular travel to partner adult day care centers throughout Northern and Central New Jersey.
