Mindify Wellness and Care is seeking a highly skilled, compassionate, and patient-centered Psychiatric Mental Health Nurse Practitioner (PMHNP) to provide comprehensive psychiatric evaluation, diagnosis, and treatment services to our diverse client population.
The PMHNP will be an integral part of our collaborative care team, delivering evidence-based pharmacotherapy and, where applicable, psychotherapy to help individuals achieve their mental wellness goals. This role requires clinical excellence, strong communication skills, and a commitment to Mindify Wellness and Care's values of empathy, innovation, and client empowerment.
Work Location & Schedule:
- Split schedule between the adult daycare centers (local travel is required):
- Perth Amboy, NJ – 2 days per week
- South Perth Amboy, NJ – 1 day per week
- Opportunity to provide additional virtual care
Key Responsibilities
Comprehensive Psychiatric Assessment & Diagnosis
- Conduct thorough initial psychiatric evaluations and ongoing assessments
- Perform psychiatric histories, mental status exams, and risk assessments
- Formulate accurate diagnoses using DSM-5-TR criteria
- Assess for co-occurring medical and substance use conditions
Treatment Planning & Management
- Develop individualized, evidence-based treatment plans
- Prescribe, manage, and monitor psychotropic medications
- Order and interpret labs and diagnostic tests
- Adjust treatment plans based on clinical response and best practices
Therapeutic Interventions
- Provide culturally sensitive psychotherapy (e.g., CBT, DBT, supportive therapy) within scope
- Deliver psychoeducation to patients and families
- Provide crisis intervention and stabilization when needed
Collaboration & Coordination of Care
- Work collaboratively with psychiatrists, therapists, social workers, and care teams
- Participate in case conferences and clinical meetings
- Coordinate referrals and community resources
Documentation & Compliance
- Maintain accurate and timely EMR documentation
- Ensure compliance with HIPAA, billing, and regulatory standards
- Adhere to ethical and professional nursing standards
Professional Development & Quality Improvement
- Participate in quality improvement initiatives
- Maintain continuing education and certifications
- Stay current with psychiatric and pharmacological advancements
Qualifications
Education
- Master’s or Doctorate degree from an accredited PMHNP program
Language Requirement
- Fluency in both Spanish and English (required)
Licensure & Certification
- Active RN and APRN licenses in [State]
- Board Certified PMHNP (PMHNP-BC)
- Active DEA registration with prescriptive authority
Experience
- Minimum of 2–3 years of post-licensure PMHNP experience preferred
- Experience in outpatient settings and diverse populations
- Experience providing telehealth services preferred
