Mindify Wellness and Care is seeking an outgoing, compassionate, and relationship-driven Patient Care Advocate to serve as a trusted presence to represent Mindify within adult day care centers throughout New Jersey. This individual will educate participants about our behavioral health services, build meaningful relationships with adult day care center attendees, support patient engagement and continuity of care, asist with translating during therapy and/or psych sessions, and help connect individuals with Mindify mental health services.

This role is ideal for someone who enjoys working directly with older adults, educating and supporting patients, building lasting relationships, and making a meaningful difference in the lives of others.

Responsibilities

  • Travel to assigned adult day care centers throughout northern and central New Jersey.
  • Deliver engaging presentations and educational sessions about Mindify's psychiatric, therapy, and behavioral health services.
  • Build strong, trusting relationships with adult day care participants, caregivers, and center staff.
  • Educate individuals about available mental health services and resources and answer questions regarding treatment options.
  • Identify participants who may benefit from behavioral health services and facilitate referrals.
  • Advocate for patients by helping them understand and access appropriate services and support.
  • Support continuity of care by helping patients attend scheduled appointments and remain engaged in services.
  • Serve as a liaison between Mindify, patients, families, providers, and adult day care center staff.
  • Maintain strong working relationships with each center's Administrator, activity staff, and clinical teams.
  • Provide Spanish-English interpretation and translation for patients, families, and providers as needed.
  • Maintain accurate documentation of patient interactions, referrals, presentations, and outreach activities.
  • Maintain HIPAA compliance and patient confidentiality at all times.
  • Perform additional patient advocacy, outreach, and support duties as assigned.

Requirements

  • Bilingual in English and Spanish (Required).
  • Excellent public speaking, presentation, and communication skills.
  • Passion for working with seniors and diverse communities.
  • Strong interpersonal, advocacy, and relationship-building abilities.
  • Comfortable presenting to groups and engaging with participants one-on-one.
  • Ability to build trust and establish meaningful connections with patients and families.
  • Valid driver's license and reliable transportation.
  • Willingness to travel throughout New Jersey.
  • Strong organizational and time management skills.
  • Proficiency with Microsoft Office and electronic systems.
  • Experience in patient advocacy, patient services, healthcare, behavioral health, social services, senior services, customer service, or a related field preferred.

Compensation

  • $23.00–$30.00 per hour, based on experience and qualifications.
