Clinical Social Worker
Mindify Wellness and Care is seeking a highly skilled, compassionate, and patient-centered Clinical Social Worker to provide comprehensive clinical services to our diverse client population. This role is an integral part of our collaborative care team, supporting individuals in achieving their mental wellness goals through evidence-based interventions and holistic care.

Position Overview
This is a hybrid position requiring both remote and in-person work, with travel among our adult day care centers across Northern and Central New Jersey. The MSW will work with individuals, families, and groups to enhance social and emotional well-being.
Essential Duties and Responsibilities
- Provide individual, group, and family therapy to clients with a range of needs, including mental health concerns, substance use disorders, and family challenges
- Conduct comprehensive psychosocial assessments and develop individualized treatment plans
- Deliver evidence-based interventions to support client progress and wellness goals
- Coordinate care with multidisciplinary providers, including psychiatrists, PMHNPs, psychologists, and nurses
- Advocate for clients and their families to ensure access to appropriate services and resources
- Provide education, guidance, and emotional support to clients and families
- Maintain accurate and timely clinical documentation and reports
- Participate in team meetings and collaborative care planning
- Contribute to the development of innovative social work interventions and programming
- Engage in ongoing professional development and training opportunities
Qualifications
- Master’s Degree in Social Work (MSW) from an accredited program
- Active New Jersey (NJ) licensure required (e.g., LSW or LCSW)
- Minimum of 2+ years of experience providing clinical social work services
- Bilingual required (Spanish and English)
- Strong clinical assessment and therapeutic skills
- Excellent communication and interpersonal abilities
- Ability to work independently and collaboratively within a multidisciplinary team
- Demonstrated commitment to social justice, diversity, equity, and inclusion
